Mammoplasty
This procedure seeks to increase the breast size in women that for some genetic reason, nursing or asymmetry have a deficit of mammary volume.This surgery will allow the patient to increase from one to two cups in brassiere size and in some occasions will be able to increase the size of it.
For 99% of my patients I recommend the use of silicone prosthetics, for quality, dependability and advantages that this product has over other prosthetics offered in the market. Serious studies have demonstrated that the appearance of breast cancer and collagen illnesses doesn't have a direct relationship with the use of these prosthetics. Apparently the early detection of masses is easier with implanted patients, since the prosthetics allow a more precise self-exam and these patients are more careful with this procedure. The precocious detection of suspicious masses through radiological exams and the training of specialists that carry them out, has vastly contributed to the safeness of these implants. The patients with implants don't have any nursing alteration neither have any limitation to carry out their normal activities, their routine exercises and any other activity type like flying in airplane, diving, etc.
Other implants I offer are Saline Solution implants broadly used in countries like United States.
The inconvenience is that they can end up losing their volume with time and present a decrease of the initially placed volume due to leakage through the valve, or lose its whole volume in a sudden manner, making necessary a surgical intervention.
It is important to mention that the silicone is a heavier element than water and to more placed volume, more uncomfortable the patient can feel.
Pre-surgical Evaluation
The initial exam will allow the Doctor to show the patient the characteristics of the breasts, the patient's alterations on their thorax, the placement of the nipple, the size of the areola, the height of the mammary fold, which will allow defining the place for the implants, its volume and place where it will be placed.It’s important to inform your general state of health, illnesses suffered, medication that has been taken, mainly those that present some alteration in clotting properties like corticoids, aspirin, and anti-inflammatory.
Discuss the type of anesthesia used, the necessity for laboratory exams and the appointment for a pre-anesthetic evaluation.
Surgical Procedure
It’s a surgery that can be carried out with local anesthesia plus sedation, epidural and general anesthesia, the placement methods that I recommend are the areola method and the infra-mammary, both have good esthetic results, however the areola method is preferred for the little definitive scar produced. The prosthetics can be placed below the gland or below the muscle and it depends on the tissues found in the initial exam. The procedure lasts approximately 1 to 1 ½ hours, and it is carried out in an ambulatory form.Post-Surgical
The patient leaves the surgery room with a special surgical brassiere that has been previously requested by the Surgeon. The patient will use it permanently during the first month day and night and I recommend continuing using it for sleeping. I think part of the success of this surgery depends on how sensible the patient is in wearing this garment to avoid the breasts falling due to weight of the prosthetics, mainly when they are placed below the gland. The pain is controlled with analgesics and anti-inflammatory medicine formulated by the Surgeon. Also use local cold compresses or ice to help control of the pain, avoiding direct contact so not to burn the skin. The prosthetics placed below the muscle are more painful and it is not strange to find some aches during the first month, mainly at night when sleeping.The stitches are removed after 15 days, but the patient will be able to renew her daily activities starting from the third day, avoiding strong exercises until the first month.
A transitory decrease of the sensibility of the nipple can occur, other times hyper sensibility is present, but this will be normalized after the second week.
Possible Complications
Hematoma and bleeding is not frequent after surgery; in 2% of the prosthetics placed below the gland and in 0.2% of those under the muscle, infection or breast hardening (capsule) occur.It is recommended to take ultrasounds and massages from qualified personnel on the 7th, 15th and 21st day after surgery.
