Breast Lift, Breast Reduction

The reduction mammaplasty is the surgery by means of which the Plastic Surgeon reduces the size of the breasts, achieving with it an aesthetic improvement and the disappearance of the overweight symptoms that affect these patients. There are many factors that affect the growth of the breasts such as hormonal unbalances and obesity are the most frequent.

The Mastopexy (breast lift) is applied to women that present ptosis (fallen breasts) for not using brassiere, the nursing, the increase and decrease of weight, the loss of elasticity of the skin, the age, etc. This surgery relocates aesthetically the breast, without diminishing their volume and in occasions when the glandular volume is very scarce, it will become necessary to place a silicone prosthetic to give a better form and enhance the shape.

These surgeries don't guarantee in any way a perfect symmetry, some are with a perfectly acceptable asymmetry.

Pre-surgical Evaluation

It should be clear for the patient in this first interview that the reduction surgery and rising of the breasts will always leave scars whose form and behavior vary according to the technique used and to the skin excess and existent gland in the patient.

At the moment the existent techniques for these two procedures leave two types of scars, one in form of inverted T and the other one in vertical form under the areola, obviously both will have a scar around the areolar complex. What determines the scar type is the mammary volume present and the ptosis of the breast. These scars usually evolve favorably and they are not very notorious, mainly the vertical ones and that of the areola, the horizontal scar in the lower mammary pleat in occasions can present a thickening that can end up becoming a queloid in occasions, entity that is characterized to be more related with the quality of the skin that with the form in that one makes the suture.

Scars produced by procedure

In this interview it is important to inform on the part of the patient their conditions of health, previous illnesses, medicationbeing taken, mainly corticoides, aspirin and their derived, antiinflamatory drugs, Vitamin E, Smoking should be suspended 2 weeks before and 2 after surgery.

Surgical Procedure

It is a surgery that is carried out usually in ambulatory form, in some cases and according to the size of the breasts a day of hospitalization it is recommended, or when the patient has some condition as the high pressure, for example.

The recommended anesthesia is the epidural, but one can also make with general and in very selected cases of rising of the breasts, local plus sedation. The duration of the surgery is on the average 3 hours.

The resulting scars, besides the scar around the areola, will have a vertical one from the inferior border of the areola until the inferior mammary pleat, in other cases besides two o'clock a scar located in the pleat mammary inferior giving a form of invested T will exist.

The patient when finishing the procedure will wear a surgical brassiere that should be worn during 1 month, day and night, and as much as possible the patient should continue sleeping with it in indefinite form.

Postoperative

The patient in the first days will be able to have some nuisance in the mobilization of the arms, besides the resulting swell of the surgery and the mobilization of tissues that take place during the procedure, this will improve following the recommendations made by her Surgeon and by taking the analgesics and antiinflamatory drugs, either local cold with compresses or ice should be placed yet protected appropriately to avoid the burns of the skin. Starting from the third or fourth postsurgical day the patient can return to her normal activities, she can change the cold for local heat, the stitches of the areola will be removed after 10 days and the remaining ones among the day 15 to 20, the scar the first three months will be red and notorious, but in the measure that the time passes it will become paleer and will take the aspect of an old groove. It is recommended to use creams that help to the scaring process, these they will begin after the retirement of the stitches, in the moment indicated by the Surgeon. When it becomes necessary the revision of a scar this won't happen before 6 months and after an appropriate medical treatment of the same one.

The gym sports, extreme physical activities will begin after 4 weeks, avoid the direct sunlight and the suntan for the first two months.

This surgery should not affect the nursing neither the sensibility of the complex areola nipple, it is possible that initially she has some alteration in the same one but it will disappear in the first weeks.

Possible Complications

The mammaplasty and the mastopexy are surgeries with very few complications, the main of them is the postoperative hematoma that makes necessary their evacuation in surgery, the infection, the wide scars and in some cases queloids. Asymmetries can be presented, but it is important to write down that the immense majority of the women don't have symmetrical breasts, the asymmetry is a characteristic in the exam of the same ones.
