 Gynecomasty

This is the pathology that affects men in which the mammary glands suffer an abnormal growth, reaching important sizes that affect the man that suffers from it. Most of the times it is presented at puberty but it can also be presented at mature ages. The main cause is related with hormonal disorders, obesity, medications and some illnesses. The main solution is the surgical one and it is recommended to young ones when the pathology affects his emotional development and his physical activity or in mature people that are also affected by it. 

 

 Pre-surgical Evaluation

In the first evaluation with the Plastic Surgeon the main component of the pathology will be determined. Let it be fat, fat and gland or only glandular. The Surgeon will also evaluate if according to the size of the pathology it is necessary to dry up skin, leaving external scars. The patient is to inform any illnesses that he/she suffers, medications that he/she is taking that can affect clotting, such as corticosteroids, aspirin it derivates, antiinflamatorios, vitamin E, etc. 

 

 Surical Procedure

It is an ambulatory procedure that can be carried out with general anesthesia, epidural or local plus sedation, according to the patient's conditions. The great majority of the times the procedure is made in combined form with liposuction plus surgical resection of the mammary gland. When their size is not very big then it is made through a scar located in the inferior border of the areola by means of which the Surgeon can extract the fat for liposuction and the glandular tissue. When the component is alone fatty and the pathology is not very big one can achieve the same results with liposuction, in this case the scar is of approximately 3 millimeters minimum; if the Ginecomastia is important in size it will be removed through a scar around the areola and a vertical one between the areola and the mammary pleat. 

The procedure lasts approximately from one to two hours, at the end of which the patient will wear a bandage or a vest that it is made especially for the surgery to make compression in the surgical area and to try to avoid bleeding after the surgery. 

 

 Post-Surgical

It is a not very painful surgery, the nuisances that result from the surgery will be controlled with analgesics and formulated antiinflamatory drugs. The first three days it is important to apply cold temperatures in form of compresses or ice, avoiding direct contact with the skin to avoid producing burns in the skin. After the fourth day it is recommended to apply heat locally with cloths dipped in lukewarm water with magnesium sulfate. The stitches are retired the tenth day. In this first post-surgical stage, strong exercises should be avoided but the patient can renew their daily activities after the third day. It is recommended to use the vest during three weeks, avoid pools and sunlight during two and a half months.
